Example scenario

An invented clinic, used to show how the work runs. Not a client story.

A part-time CTO for a growing clinic group: the first 90 days

An invented four-branch clinic group gets a part-time CTO. The first 90 days: audit, roadmap, vendor and build decisions, compliance checks, AI opportunities and the team.

Service
  • Fractional CTO
  • Technical consultation
For
  • Clinic groups (multi-branch)

The problem

The example: Example Care Group (invented): four branches, two in Dubai, one in Sharjah and one in Abu Dhabi. Three different booking systems, an IT contractor per branch, and a records-system contract up for renewal.

The group grew from one clinic to four. Each branch came with its own booking system, its own IT contractor and its own spreadsheets.

Vendor renewals, AI sales pitches and a missing integration land on the owner's or medical director's desk, with nobody technical to ask. A full-time CTO is more than the group needs today.

What we build

  • A part-time CTO for the group, a few days a month, working with the owner and the medical director. Willow is led by a health-tech CTO with 19+ years in software and e-health.
  • One map of every system, vendor, contract and data flow across the branches.
  • A 12-month technology roadmap with priorities and budgets, in plain words.
  • Vendor and build decisions: what to keep, replace, connect or build.
  • Compliance checks: NABIDH and Malaffi connections, UAE health-data rules, the DHA AI policy.
  • A shortlist of AI uses worth trying first, and who on the team runs each one.

How it works

  1. 1

    Listen

Interviews with the owner, the medical director, branch managers and reception: where time goes, what breaks, what is planned.

  2. 2

    Audit

Every system, vendor, contract, login and data flow, plus security and who has access to what.

  3. 3

    Decide

Keep, replace, connect or build, for each system, with the reasons written down.

  4. 4

    Start

The first two projects begin, each with an owner, dates and a budget.

  5. 5

    Review every month

A monthly review with the owner: what moved, what is next, what needs a decision.

What it looks like

Example
  1. 1

    Weeks 1–4

    Understand

    • Interviews in every branch
    • Map of systems, vendors and contracts
    • Data, access and security check
    • NABIDH, Malaffi and data-protection gaps
  2. 2

    Weeks 5–8

    Decide

    • 12-month roadmap with the owner
    • Records-system renewal: keep, replace or connect
    • Build or buy: the patient WhatsApp line
    • AI shortlist checked against the DHA AI policy
  3. 3

    Weeks 9–12

    Start

    • Branch booking systems feed one daily report
    • WhatsApp reminders in one branch, then all
    • Team plan: one in-house IT lead, contractors for the rest
    • First monthly tech review with the owner
Example: the first 90 days for an invented four-branch clinic group.

What you would measure

This is an example scenario, so no results are claimed. In a real group, we would track:

  • Number of systems and vendors, and what each one costs
  • Hours of manual work between branches (copying, re-typing, reconciling)
  • Time to get one group-wide report
  • Open compliance gaps, and when each one closes
  • Decisions made on time versus decisions waiting

Rules it follows

  • Patient data stays in the group's systems. The audit looks at access, flows and settings, not patient records.
  • UAE health-data rules and NABIDH / Malaffi requirements are checked for every system decision.
  • Any AI use is checked against the DHA AI policy: patients are told when AI replies, and a person can take over.
  • Nothing is signed, switched off or sent without the owner's approval.

What AI finds in 12 months of clinic data

We ran an AI analysis on 12 months of sample data from an invented two-branch clinic. Here is what came out, and what a medical director would do with each finding.
